CANCELLATION 

You may cancel your membership by completing a cancellation form (PDF) and faxing it to 526-7672.  Please call and verify that your fax was received at 526-2222.  You may use your SAP or Student ID number in place of your SSN on this form.   

We do not accept cancellation by phone or email.
